Belmont Hospice is an expanding hospice agency striving to exceed the

Responsibilities including but not limited to:
-Establishing contact and maintaining rapport with patient/family/caregivers
-Visiting patients in accordance with plan of care, assessing for needs (DME, Medication, Change in Condition etc), and completing all necessary documentation as required by Medicare COPs
-Maintain communication and report ongoing condition of patients to office and supervisory staff, and complete IDG notes, POCs, CTIs etc. in accordance with Medicare COPs
Hospice experience is preferred, and pay is very competitive DOE!
