POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services,
coordinate admission of patient and ongoing plan of care.

ESSENTIAL FUNCTIONS

* Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME
* Case manage total care of patient by coordinating with team members/physicians, supervising C N A activities and participating in staff and Interdisciplinary Team meetings
* Complete daily paperwork and charting, thoroughly and accurately according to Policy
* Participate in on-call rotation
* May assist with admissions

POSITION QUALIFICATIONS

Registered Nurse in the state of Texas

Hold a valid driver’s license and have a dependable means of transportation

KNOWLEDGE/SKILLS & ABILITIES

Education:

* Registered Nurse

Experience:

* 1 year of nursing experience
* Case Management experience (hospice)

Strongly prefer:

* 2+ years of experience in hospice case management

Computer Skills:

Proficient skill using a computer is required; Microsoft office suite
experience preferred.
