The Client Care Manager (CCM) delivers on our mission to change the way the
world ages by making positive, powerful impacts on clients, their families and
our community.

You will join our team to provide older adults with quality care that enables
them to live happier, healthier lives at home. Our services are distinguished
by the caliber of our caregivers, the responsiveness of our staff and our
expertise in Live-In care. We embrace a positive, balanced approach to aging
centered on the evolving needs of older adults.

We provide the BEST training for client care management in the industry. We
train you in our proprietary Balanced Care Method and Cognitive Therapeutics
Method.

Once hired, our initial training process takes 4 weeks. Once trained, you
will be a highly effective and productive member of our local, experienced
winning team! Your successful contributions will be rewarded with salary and
title will increases as you and the team as the company grows. We have a fixed
career path so that your salary will increase along with the team.

You will be able to grow into a compensation of over $80-$100,000per year
over the next 2 – 5 years depending on your performance.

Home care is a PRIVATE PAY business because our services are non-medical so
we do not get reimbursed from insurance companies. Business oriented sales
experience is a CRITICAL SUCCESS FACTOR. Our successful Client Care Managers
are skilled in guiding families in their budgeting process to create a shared
vision of a path forward to care for their loved ones.

Home Care Assistance trains all of our care managers and caregivers in the
Cognitive Therapeutics Method and the Balanced Care Method, which promotes
healthy nutrition, mental and physical activities, provides extensive
caregiver training and pays our caregivers more in order to deliver a premium
level service.

The CCM is a high profile, leadership position. As CCM, you will function as a
hands-on Client Care Manager practitioner and become the local face of the
business. The CCM serves as a key point of contact for clients, referral
sources, and the broader community with a dual focus on i) supervising and
ensuring delivery of HCA’s brand of premium client care; and ii) developing
new clients/referral sources. The CCM will ensure the satisfaction of new and
existing clients by supervising and coaching care teams to use our Balanced
Care Method, Cognitive Therapeutics Method and Hospital to Home Care services
to provide premium level services.

The CCM must be available to our clients, referral sources and internal staff
during regularly scheduled On Call rotations and as needed for nights,
weekends and holidays. The CCM’s responsibility can be broken down into three
main clusters – client acquisition, client retention, referral source
marketing.

Required education:

* BSW with 4 years experience or MSW or LVN with 4 years experience or RN

Preferred experience:

* Home Care or Health Care related field: 3 years
* Customer Service: 3 years
* MSW: 1 year or RN: 1 year
