Qualifications:
- Current Registered Nurse (RN) license in good standing
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) candidates with relevant experience considered
- 2-3 years of nursing experience preferred, with a focus on community health, preventative care, or public health
- Strong interpersonal and communication skills for interacting with a wide variety of populations
Responsibilities:
- Conduct community health assessments to identify health needs and risks, and develop plans to address them
- Administer vaccinations, perform health screenings, and provide preventative healthcare education to the community
- Collaborate with local agencies, healthcare providers, and community organizations to optimize public health initiatives
- Monitor and report the incidence of communicable diseases, and participate in infectious disease control efforts
- Provide case management services, including tracking patient outcomes and coordinating care with other healthcare professionals
