How does a work-from-home nursing job differ from case management nursing?

The answer is “relative” as there are Nurse Case managers at the hospital. I do not know if there are Nurse Case Management jobs that does not require the Nurse to see the patient once every week, hence the job is “not work at home”. For example; Hospice Nurses’ title nowadays is “RN Case Manager” and they can have from 12-19 cases (Hospice patients) that they have to visit and see each one every week. The patients are either in their own homes or they are in a Long Term Care facility.
I know their job well as I talk to them all the time when they visit their patients at the LTC facility I work as Administrator.
