Usually, determining the presence of P waves, if the reading isn't super clear, heart blocks, arrhythmias, but as a nurse, you are not the only one looking at this. In most situations, a cardiologist, or rapid response team, is making final determination. I recommend reading Dale Dubin's Rapid EKG to get a really great understanding and familiarity.
