What's the protocol for reporting a malfunctioning instrument?

We have a tagging process that the instrument is tagged with the information that is the issue and the department charge person if follow up is needed. Also our department charge nurse likes to be emailed as well

It often depends on where you work but at a minimum report it to your supervisor and/or manager. Additionally, include specific details about issues and include the serial number, or ID number of the specific instrument. We also take it out of service and tag it with the above information including date and time as well as contact info for the reporter.
