What's your preferred method for assessing patient orientation?

When you obtain your 2 patient identifiers, can you tell me your name and date of birth? Then ask, can you tell me a little bit about why you are here, or can you tell me where you are at and why you are here? If they seem to have difficulty giving you answers or the answers may not make sense, or they seem confused then you can ask more questions like do you know what year it is, or who is the current president? Even if someone is at their bedside, ask them who did you bring today, or who do you have here with you?

Can you tell me your name and DOB? Do you know where we are right now? Can you tell me why you are here? What is the month and year? Do you know who the current president is? Also, a normal conversation will tell you a lot.
