What do med-surg nurses do?

Because the work of a med-surg RN is so versatile, it can be hard to compile a comprehensive list of all possible cares and duties. The following list is many of the most common roles a med-surg RN may perform, however, it is not all-inclusive.

  • Admitting, transmitting, and discharging patients
  • Monitoring vital signs
  • Administering medications
  • Operating and maintaining medical equipment – IV lines, feeding tubes, catheters, oxygen tubing, etc.
  • Documenting patient care, needs, and progress
  • Communicating and collaborating with the care team
  • Carrying out patient orders and plans of care
  • Supporting and educating patients and family members
  • Running and/or ordering tests and assessments
  • Filling out facility required education, reports, and charting
  • Working with wound care (particularly if facility has no wound care RN)

What is a med-surg care unit?

Like most RNs, med-surg nurses have plenty of autonomy. However, there is an extensive care team often involved in med-surg patient care. This multidisciplinary model has been proven to improve patient outcomes many times. Effective collaboration and communication are essential to these positive patient outcomes. The med-surg care team may include the primary care physician (PCP), an internist, a surgeon, physical therapists, occupational therapists, technicians, social workers, and case managers.

Where do med-surg nurses work?

Medical-surgical nurses work in a variety of care settings. While an acute care hospital is the most commonplace of work, you will also find med-surg RNs in inpatient clinics, health maintenance organizations (HMOs), education positions, outpatient or ambulatory care centers, nursing homes, military facilities, home health care facilities, or outpatient surgical centers. Because of the variety of institutions, a med-surg RN may work at, per diem, PRN, part-time and full time (8-, 10-, or 12-hour days) opportunities are all available.

How do you become a med-surg nurse?

Education

Associate Degree Nurse (ADN)

To practice as a professionally licensed RN, the minimum one must achieve is an ADN and pass the NCLEX-RN exam before being eligible for licensure and subsequent employment. ADN programs typically take two years to complete (after 1-2 years of the appropriate prerequisites). ADN programs are usually offered through community colleges and include coursework online, in the classroom, lab work, clinical rotations, and practicum experiences. Most students who choose to pursue an ADN degree do so understanding that they may have to back to school for a BSN.

Bachelor of Science in Nursing (BSN)

To maintain competitive advantage as well as meet the IOM mandate, most RNs choose to pursue a BSN program to begin their professional practice. BSN programs usually take four years to complete. These programs are offered at brick-and-mortar campuses and online. Much like an ADN program, they are a combination of classroom work, lecture, labor work, clinical experiences, and practicums.

Master of Science in Nursing (MSN)

This is not required for professional med-surg nursing practice. However, many nurses choose to complete their MSN, as this will allow for more professional promotion opportunities. An MSN also opens the door for teaching opportunities, higher pay, and greater responsibilities. Occasionally, some employers will reimburse or offer educational assistance for these academic pursuits.

There are also โ€œbridgeโ€ programs, which take anywhere from 12-24 months, that allow a student to move from an ADN to a BSN or a BSN to an MSN. There is also the RN to BSN programs that allows students work as they pursue their degree. As mentioned above, most ADN professionals will need to pursue higher education, often through one of these programs.

What are the additional requirements for med-surg nurses?

There are a variety of additional certifications a med-surg RN can pursue. Three of the most common are discussed below.

Medical-Surgical Nursing Certification (RN-BC)

This certification is accredited by the Accreditation Board for Specialty Nursing Certification. It is a competency-based exam that assesses entry-level clinical knowledge and skills of the RN in the med-surg specialty. It must be renewed every five years.

Certified Medical-Surgical Registered Nurse (CMSRN)

This certification is the recognized path to prove excellence and commitment to the field of med-surg nursing. It is considered a prestigious credential, currently held by over 38,000 professionals. Recertification is necessary every five years.

Certified in Care Coordination and Transition Management (CCCTM)

This credential demonstrates an RNโ€™s knowledge of patient-centered assessment and care planning. It was created in collaboration with the American Academy of Ambulatory Care Nursing. New applications for the certification are currently on hold as the board of directors evaluates the program.

What does a temporary nurse aide do?

A temporary nurse aide administers many important tasks throughout their shift to assist the nurses on staff. That said, they have to work under the supervision of a registered nurse or nurseโ€™s aide.

How do you become a temporary nurse aide?

To become a temporary nurse aide, you must complete an eight-hour online course through the American Health Care Association. The course covers daily living, infection control, and prevention, positioning, nutrition and elimination, along with comfort and end-of-life care.ย 

Candidates must answer 40 of 50 questions correctly to pass the final assessment. After passing the course, candidates will receive a certificate that they can give to their potential employers.

The certificates expire on April 21, 2021. The certificates are suitable for the pandemic duration or at least while there is a public health emergency in place.

Temporary nurse aides can find employment with nursing homes or hospital facilities with their certificates.

How can you convert to a certified nurse aide?

To go from a temporary nurse aide to a certified nursing assistant, short-term aides must transition into an emergency nurse training program. The emergency nurse aide training program requires at least 75- hours of training. 

However, temporary nurse aides can apply their eight hours of initial training toward the 75-hour minimum requirement for certification. Also, 16 of the training hours have to involve a clinical setting.

The 1135 waiver (related to nursing home) for temporary nurses in training is set to end on June 6, 2022. This means that temporary nurse aides in training will have to return to the 4-month course and be completed by October 6, 2022.

What are the job duties of a nurse practitioner?

Nurse practitioners are advance practice registered nurses (APRN). They work independently to manage patient’s health and to prevent disease. NPs must earn an advanced degree: either a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP).

The advanced degree lets them have greater authority (and responsibility) than a registered nurse.

Nurse practitioners perform many of the same duties that a physician would do. They visit patients, diagnose and create treatment plans. Nurse practitioners also work with patients who have chronic conditions to help manage the situation and make necessary changes. 

Additional duties involve:

  • Managing other nurses and staff within the clinic
  • Ordering diagnostic tests for patients
  • Prescribing medicine to patients
  • Administer routine and detailed examinations
  • Explaining complicated medical and treatment plans to patients and family 
  • Analyzing tests results to discover causes for symptoms

What are the job duties of a registered nurse?

Registered nurses act as primary caregivers to patients. They perform the following duties: 

  • Administer IV medications

RN vs NP: Similarities between both roles

RNs and NPs both share an ability to specialize and work in diverse environments. 

Some of the most common RN positions include ICU nurse, NICU nurse, and Med-Surg nurse.

A few of the most common NP roles include Family Nurse Practitioner, Psychiatric Nurse Practitioner, and Neonatal Nurse Practitioner.

RNs and NPs assist patients in medical settings such as hospitals, urgent care clinics, or long-term care facilities. And in both roles, the nurses work directly with the patient and their families. However, RNs typically work in more types of settings than NPs. 

Most nurse practitioners find themselves in a clinical setting while there are some registered nurses who work on cruise ships.

RN vs NP: Differences between both roles

Salary differences between RNs and NPs

There’s a significant difference in salary between Registered Nurses and Nurse Practitioners.

Registered nurses earned an average annual salary of $82,750 in 2021, according to the Bureau of Labor Statistics (BLS).

Nurse practitioners earned an average annual salary of $118,040 in 2021, according to the BLS. 

Why is such a significant difference? The reason why NPs more money than RNs nurses is because they have an advanced degree.

Also, a nurse practitioner’s role is more expansive than that of a registered nurse. The nurse practitioner has much more responsibility and autonomy than the registered nurse.  In some states, NPs work autonomously and have their own practices.

Educational requirements for RNs and NPs

One big difference between an RN and an NP is their level of education.

  • Registered nurses need either an associate’s (ADN) or bachelor’s degree (BSN).
  • Nurse Practitioners, meanwhile, need a Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP). 

Both roles require clinical training.

  • RNs generally need at least 400 hours of clinical experience.
  • NPs typically need about 500 or more hours.

Continuing Education Requirements for RNs and NPs

RNs and nurse practitioners both take continuing education courses to retain licensing. 

Most registered nurses have to complete continuing education units (CEUs) in order to maintain their licenses. Continuing education requirements vary by state and specialty.

For instance, California requires RNs to take certified courses adding up to a total of at least 30 contact hours

All nurse practitioners have to complete CEUs. They must renew their license every five years with the American Association of Nurse Practitioners. To renew, nurse practitioners need to accrue 1,000 clinical hours since their last renewal along with 75 contact hours of CEU.

Licensure and Certification for RNs and NPs

Registered nurses must be licensed in the state they live in in order to work there. Certification standards differ depending on the state. But most have a few basic requirements for certification:

  • Earn an associate or bachelor’s degree at an accredited institution
  • Pass a background check
  • Pass the NCLEX exam

Once RNs receive their licensure, they can obtain certification in the specialty of their choice.

For NPs, licensure requirements are mostly the same as RNs. The difference? Nurse practitioners need to receive their certification through either the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.

NP vs. RN: How to choose the right path for you

Pursuing a career as a nurse practitioner or registered nurse has its own unique set of benefits and drawbacks.

While nurse practitioners make more money, the role also comes with more responsibility, and in some situations, complete autonomy.

That responsibility can also lead to more significant burnout.

Accordingly, the registered nurse role might seem less stressful in scope. On the other hand, some people excel in leadership roles with greater responsibility. It also doesn’t require additional schooling beyond an ADN or BSN.

What is a DNP degree?

The Doctor of Nursing Practice is an advanced degree within the nursing field. It is one of the two terminal โ€” or highest โ€” degrees a nurse can achieve.

DNPs who choose to become advanced practice registered nurses (APRN) have an opportunity to work directly with patients by managing, evaluating, and assessing treatment and care.

NP vs. DNP

The main similarity between an NP and a DNP is they are both advanced positions. In other words, they both require a graduate degree.

Additionally, these two positions share a lot of the same job opportunities. Nurses who receive graduate degrees can become many different types of advanced practice registered nurses. Some of these include:

How to get a DNP degree

How do you get a DNP degree? There are multiple ways to receive a DNP. Often, people get their BSN to MSN and then a DNP. However, there are bridge programs to help you achieve your doctorate and save time.

MSN to DNP

This MSN to DNP bridge program is the most common method of getting a DNP degree. Since the nurse will already have a Master of Science in Nursing, they most likely will work in an advanced position. This means they may not have the capacity to go to school full-time.

The MSN to DNP program takes 1-2 years full time or 3-4 years part-time. This nursing program revolves around specialized coursework. Clinical hours from the MSN program may apply toward the clinical hour requirements.

BSN to DNP

The BSN to DNP bridge program brings together the curriculum of an MSN and DNP. A few programs allow you to leave the program once youโ€™ve completed the MSN requirements.

If you choose this program, expect it to take 3-4 years full time or 4-6 years part-time. The program includes MSN and DNP curricula to avoid repetition. It often consists of an early exit before the DNP portion.

RN to DNP

The RN to DNP bridge program is less common. However, it allows someone without a bachelorโ€™s degree to pursue a DNP.

An RN to DNP program usually takes 4-6 years full time or 6+ years part-time. Some programs require prerequisites to take undergraduate coursework before taking graduate courses. You also have the potential to earn an MSN degree before taking DNP courses. Other programs provide the option to award an MSN and DNP after graduation.

Direct-entry DNP

The Direct-entry DNP program is a good option for students who have a bachelorโ€™s degree in an area outside of nursing. The program takes 4-6 years full time. Part-time options are generally not available. Students have to become registered nurses before taking graduate coursework

John Hopkins University

Johns Hopkins University, a private school in Baltimore, Maryland, is well-known for its nursing programs. The schoolโ€™s DNP degree is no different. The application deadline is January 1st and there is an application fee of $75 for U.S. residents.

Rush University

Rush University is a private school located in Chicago, Illinois. If youโ€™re interested in pursuing a DNP degree at this school, the application deadline is January 2nd. Prospective students who are U.S. residents must pay an application fee of $115.

The University of Washington

A public school in Seattle, Washington, the University of Washington has an impressive School of Nursing. The application deadline is January 15th. U.S. residents must pay an application fee of $85.

Duke University

Duke University is a private school in Durham, North Carolina. If you decide to get your DNP degree here, apply by December 1st. The application fee is $50 for US residents.

Columbia University

Columbia University in New York, New York is a private school with strong nursing programs. This school has an application deadline of January 20th and an application fee of $75 for U.S. residents.

How much does a DNP program cost?

You can expect to pay between $21,318 and $38,912 for an MSN-DNP program. A BSN-DNP will cost between $40,953 and $74,752 for your tuition.

These numbers might seem intimidating, but donโ€™t let them scare you off. Every nursing program is different. And there are several steps you can take to get help paying for school.

Paying for a DNP program

Once youโ€™ve selected the best program for you, youโ€™ll need to decide how you want to pay for your DNP degree.

Take inventory of your financial situation and see if you qualify for government funding. To find out if youโ€™re eligible, fill out the Free Application for Federal Student Aid (FAFSA). Schools use this tool to determine eligibility for providing need-based grants, scholarships, and student loans. Is a DNP higher than a PhD?

Neither a DNP or a Ph.D. is necessarily โ€œhigherโ€ than the other. The Doctor of Nursing Practice focuses on nursing practice, while a Ph.D. in nursing focuses on nursing research. While both degrees are doctoral level, graduates will be working in different capacities after graduation, with different roles and responsibilities.

Is a DNP the same as a nurse practitioner?

No. DNPs and NPs are different. The DNP is an advanced nursing degree that qualifies graduates to work in a variety of nursing specialties. NPs earn a masterโ€™s degree and can work as primary care provider.ย 
However, these are both advanced nursing positions. Nurses who are qualified to work as DNPs or NPs may be eligible for the same types of roles. And if a DNP becomes an advanced practice registered nurse (APRN), they will be able to work as a nurse practitioner.

It may feel like a daunting task to make the transition. When you went to school, you may have learned how to insert and take out a trach care, but what about writing a resume? Or even a cover letter? What in the heck does an NCLEX stand for and why is it important?

The good news is that we have some practical tips to help you transition from nursing student to a new grad in no time.

How do I obtain my ATT?

The ATT or authorization to test is a way for the national board to determine if you are qualified to take the NCLEX (RN or PN). The qualifications vary by state. Generally, they include passing a criminal background check and valid identification. Transcripts demonstrating that you successfully graduated from nursing school are also required. You should receive your ATT within a month after filling it. The ATT is good for 90 days after you receive it in your email. 


How do I apply for jobs?

As youโ€™re waiting to receive your ATT, itโ€™s time to apply for jobs. Applying for jobs can feel overwhelming. You might not know where to apply or even how the application process works. First, you need to figure out what type of job you want in the nursing field. Do you want to work bedside? Do you want to work in a hospital, a doctorโ€™s office or a school? There are a lot of options out there for you. Once you choose where you want to work and what job you want to have, you can start applying. 

Probably the most important step in trying to find work is having a good resume and cover letter. Once you get your resume and cover letter in shape, the next step is to go on job boards such as Incredible Health to find a job thatโ€™s suitable for you. 


What is the best way to study for the NCLEX?

The NCLEX test gives you the green light to start working as a registered nurse.

The test is made up of 75 to 265 questions. It may sound like a lot. But you have 4 hours to complete it. The test covers the following areas:

  • Safe and effective care environment
  • Health promotion and maintenance
  • Psychosocial integrity
  • Physiological integrity

Next are some study tips to help you ace the exam!

Don’t cram!

  • This is a marathon, not a sprint.
  • It’s too much information to try and โ€œrefreshโ€ in just a couple days.

Have a system 

  • You can use a test prep service (ATI, Kalpan), especially if it is offered through your school.
  • A system gives you more structure.
  • Get tips on how to get through all the modules what to work on.
  • Break it down by specialty (such as OB, Mental Health, Med Surg).

The more questions you can look at and answer the better! 

  • Get comfortable with all types of questions (such as multiple choice or “select all that apply”).

Study your way

  • Study the way that works best for you. It could be in groups, a pair or alone.
  • How or who study with doesnโ€™t matter โ€“ just make sure to do it the way that works for you! 

Take it easy before test day

  • Take the night off. Go to bed early so you’re well rested.
  • Don’t try to study the day of the exam. Focus on relaxing and getting to the testing site early.

What state do I take my NCLEX in?

One of the first steps after you graduate should include finding out what state to take your NCLEX in. The NCLEX stands for the National Council Licensure Examination. This test is mandatory for anybody considering working as a registered nurse. You can take your NCLEX in any state. 

Just know that you will have to go through the process of transferring your license if you want to work in another state. Ideally, you want to get your NCLEX in a state that youโ€™re planning on living in. However, if you are going to be moving to another state in the near future it may be good practice to take the NCLEX in the state youโ€™re moving to.
