What's the difference between actual problems and potential problems in nursing?

I’ll give an example- ventilated patient.
Actual nursing problem- ineffective airway clearance (pt cant clear their secretions with a barrier), Anxiety (pt panics when they wake up)
Potential problems- risk for impaired tissue integrity (the pt might not have it yet but because the pt is ventilated therefore has limited mobility and nutrition, the pt had the risk for pressure ulcers)
basically actual problem is a problem you can see/prove through your nursing assessment, while potential problem can occur anytime due to patient’s risk factors
