I’ve been a nurse for 1.5 years, all of which I have been working in psych and detox. I’ve been trying to get into medical (med surg), but no takers except a SNF. Would working in a SNF hurt my chances of getting into med surg later?

No, I actually worked at a SNF as a new grad, got hired into med-surg/telemetry after a year of working at the SNF, then ICU. Definitely able to do it!
