What are the duties of a case manager?

Case Managers responsibilities vary, that is whether you are in a hospital or Home-care setting. Here’s a brief summary.

Case Managers perform several tasks /duties that usually involve working with their clients directly and their client’s significant others. A holistic approach.

Assessing clients’ physical and mental wellness, preferences and abilities and using these and other needs to develop tailored care plans
Communicate with clients, and their support networks and health care professionals to implement care plans
Listening skill (very important) to clients’ concerns in providing counseling or intervention as required
Recording clients’ progress, charting referrals, home visits, medical equipment being utilized and other notable interactions
Evaluating clients’ progress periodically and making adjustments as needed to improve outcomes
Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mental health. I was a Home-care Case Manager and would see 5-7 patients some days with responsibilities that include wound care, drainage devices and more.

Csse management varies depending on the setting. Homecare case management involves managing a group of assigned patients, complete assessments and re-evaluation, med/disease management. Usually you work with interdisciplinary team. Hospital setting is more about discharge planning and connecting them with resources

A case manager oversees the medical case of the patients under his/her care. This is done by coordination of Providers, procedures, insurance issues, appointments, education, and above all good communication with all team members, patients and families.
